Welcome to the Terre Haute Testing Centers Test Request Website.

If you are enrolled in a Terre Haute based Distance Education class that requires a proctor, you will need to set up a proctor, select the class, and order your exam on this website.

Logging into the website: You may use any login password you wish. We recommend that you use your campus connect login. This website is not associated with Blackboard or Campus Connect so each person will have to create a new record. These accounts are cleared at the beginning of each semester.

Setting up your proctor: You have two options:
1. Choose one of the pre- approved proctors from our list or
2. If you live outside of Indiana or at least 45 minutes from one of the pre-approved sites, you may submit your proctor.

PROCTOR GUIDELINES:
Students in the Terre Haute vicinity will take their exams in the Testing and Assessment Center located at the Hwy. 41 South campus.

If a student is in the vicinity (45 minutes driving distance) of another Ivy Tech State College site, the student will take exams at that Ivy Tech approved site.

Students, not located within Ivy Tech State College vicinity, will be responsible for selecting a proctor to administer the course exams.

The submitted proctor must be capable to receive the exam via email in a Microsoft Office Products format and have the ability to proctor a web based exam.

The submitted proctor must also meet one of the following requirements:
? a librarian at a public or college library
? a full time faculty member or academic staff member at a regionally accredited college or university
? a full-time, state certified elementary or high school teacher
? an education officer on a military base or any commissioned officer of higher rank than the student
? a minister or priest
? a personnel executive (HR) of the corporation providing your tuition aid.

Friends and relatives are not permitted to be proctors.

Selecting your exam: Once your proctor is verified you need to add your class to your account. The classes are listed by course ID and section number with the instructor’s name. If you choose the wrong class, add the correct one and contact our office to have the incorrect one deleted. The student is responsible for selecting the correct class and order the correct exam.

Ordering your exam: After you select your class you are ready to order your exam. You may log back on to this website to check the status of your exam. As soon as we email the exam to your proctor, the website will show the date and time the exam was sent. Students must allow 72 hours processing time for exams. It is the student’s responsibility to contact the proctor and arrange a time to take the exam.

If you order the wrong exam or wait until the last minute to order your exam your request may not arrive in time to meet the deadline.
